Homecare Facts For Physicians

Homebound is defined as:

Individuals who cannot leave home regularly without difficulty and without assistance are considered to be homebound.

Services and programs are based on the needs of your patients. VNS can provide:

• Skilled nursing Home infusion therapy
• Physical, occupational, and speech/language therapies
• Home health aide assistance, including personal care
• Medical social services
• Geriatric case management
• Nutritional and dietary guidance/education and/or Meals On Wheels
• Pain and symptom management/Palliative Care
• Hospice care

Start of care can be initiated usually within 24 hours of receiving your home care services referral.

It’s easy to refer your patients to VNS for home health services or hospice care.

Hospice Care

For hospice care, call 585.787.8315 and speak with our Intake Nurse, or contact one of our Home Care Coordinators at any one of the hospitals. Download a Hospice Referral Form, complete and fax back to us at 585.787.9728 with a current medication list for your patient. Our triage staff will call you to confirm receipt of referral.
