Program Eligibility
Making a long-term healthcare decision for yourself or a loved one can be an emotional and overwhelming experience. At VNS, we can help make that decision easier. Our LTHHCP provides the care and assurance you or your loved ones need to be able to stay at home in familiar and supportive surroundings.
Referrals may be made by a member of your family or other community service representatives. Arrangements may be made before discharge from the hospital, a long-term healthcare facility, or directly from the community. To be eligible for the program, you must:
- Have Medicaid coverage and insurance
- Reside in Monroe County
- Be medically eligible for nursing home level care, either skilled or health related
- Obtain your physician's approval for long-term home healthcare
- Be safely cared for at home, based on a complete health, social and environmental assessment
Insurance Coverage
The cost of services is covered by Medicaid. Some services are covered by Medicare and private insurance plans, but you must be covered by Medicaid Insurance to be eligible for the LTHHCP.
