Together we’ve expanded hospice care in the region by creating the Palliative Center

for Caring, developed to provide you and your family with an atmosphere of comfort

and distinctive levels of care.

 

A first in the Rochester area, the center is the result of our strong collaboration. Located on the second floor of The Heritage, our five-bed unit offers hospice care to those who can benefit from a specialized hospitalization environment, while providing closeness with family and loved ones.

 

The Comforts Of Home
 

What we offer at the Palliative Center for Caring includes:

 

  • Private patient rooms, beautifully decorated, featuring a TV and DVD player, telephones and private baths
  • Around-the-clock visiting privileges for family members, including overnight accommodations
  • A cozy living/lounge area with a fully equipped kitchen, dining area and computers with internet access
  • Food service from The Heritage for family members
  • Access to St. Ann’s Campus common areas like a greenery-filled lobby, café, library with internet access, chapel and peaceful walking paths
  • Physician supervision and coverage available for all patients
  • Special support services available as needed

 

The Patient Experience: Comforting Surroundings

 

Patients are admitted to the Palliative Center for Caring for:

 

  • Short-term pain and symptom management
  • End-of-life care
  • Typically three to seven days of care
  • Devoted care to the quality of life, ensuring dignity, meaning and peace

 

An Experienced Team with One Goal

 

The staff is comprised of specially trained professionals from St. Ann’s Community and visiting clinicians from the Hospice Care program at VNS, including:

 

  • Clinical Nurse Specialists
  • Physical, Occupational, and Speech Therapists
  • Medical Social Workers
  • Dieticians
  • Home Health Aides
  • Pastoral and Counseling Professionals
  • Volunteers
