Giving the Gift of Hospice

At Visiting Nurse Hospice & Palliative Care, our patients and their families always come first. We know that comfort begins by staying in familiar surroundings, being cared for by caregivers who become like family. We provide quality care and compassion to those that need it, including those who may be unable to pay.

Frequently, insurance reimbursements do not cover the true cost of the service. Your thoughtful contribution and donation – made in memory of loved one – will ensure that we can provide the support patients and family members require. You can help continue this tradition of comfort and compassion for all who need it through the Visiting Nurse Foundation, a tax exempt, not-for-profit corporation. We welcome the smallest of gifts, for it comes with the greatest intention.

Wish list items needed to enhance patient care delivery or provide peace of mind include:

Lasting Memories: Help purchase a small, lightweight camcorder for volunteers to use in recording patient video histories – $500

Caregiver Support: Provide one day of respite care for a family member by hiring a private aide to assist with patient – $100

Caregiver Basket: Send a basket filled with gift certificates for a restaurant or a massage; or nice writing pens, paper, and a Caregiver Booklet; or snacks/parking passes – $100

Special Occasion Meal: Provide a home elivered meal for a patient and family – $100

Music Therapy with (or without) Massage Therapy: Purchase relaxation tapes or soothing music, portable CD or cassette players – $100

Comfort Care Supplies: Provide room air onditioners, table fans, wedge pillows, cotton jersey sheets, warm clothes, or lightweight blankets – $50

Bouquet of Flowers: Send a floral arrangement to a patient or caregiver – $30 to $50

Tabletop Fountain: $50

Personal Care Pack: Provide the smallest of life’s necessities, like tissues, lotion, socks, etc. – $25

Call Me: Purchase a 60-minute calling card for patient use – $20

Gas Cards for Families: $20

Wind Chimes: $15

Other suggested items that will help raise funds for programs and services or offset expenses include:

  • Donating a vacation home or rental property – used for Hospice Gala or Golf Tournament auction item.
  • Donating gift cards for grocery stores with pharmacy services for use by patients’ families who are having trouble paying for groceries or prescriptions while caring for a terminally ill patient.
  • Donating gift certificates to area stores or restaurants for staff members or patient families.
  • Donating theme park, attraction or movie tickets for patient families.
  • Donating soft fabric for patient pillows, and pillow filler (soft, washable) for volunteers to make pillows with.
  • Donating yarn for lap blankets.

Visiting Nurse Hospice and Palliative Care

Offering caring and comfort through life's final stage. Dying and death, as the final stages of life, present terminally-ill individuals, their families and caregivers with many new and difficult challenges. At the same time, there are opportunities for compassion, growth and understanding unmatched at any other life stage. The Visiting Nurse Hospice and Palliative Care Program supports patients and their loved ones throughout this most difficult time, providing professional home care that meets both medical and emotional needs, promoting as full and comfortable a life as possible.

Our hospice and palliative care professionals are experienced in every aspect of hospice care, from guiding the patient's smooth transition from hospital to home…to providing comfort care with respect and compassion…to supporting family and friends through their bereavement following death.

At Visiting Nurse Hospice and Palliative Care, we believe each individual has the right to choose, insofar as possible, the extent of his or her medical treatment. We work closely with your physician to ensure that symptom and pain relief, as well as sensitivity to emotional needs, are at the center of all we do.

Palliative Care Services
Palliative care is the comprehensive management of patients' physical, psychological, social, spiritual and existential needs. It can be part of the treatment of any person with a serious or life-threatening medical condition for which a patient-centered approach, pain and symptom control, family involvement and compassionate care are needed.

One need not be dying to benefit from palliative care. It is valuable at any time during a serious illness. Hospice is one form of palliative care. And like hospice, palliative care can be provided in a variety of settings including the hospital or the patient's home.

The intensity and range of palliative intervention may change as an illness progresses and the complexity of care increases. Our services are available in-home and on an out-patient basis through our Palliative Care Consultation Service. Our Family Nurse Practitioner is certified in palliative care and works collaboratively with the physician to make recommendations in regards to symptom management, advanced care planning, clarifying goals of care and serves as a link to other community services as needed.

Hospice Services
Eventually, the focus shifts to the process of dying, with emphasis on end-of-life decision making and achieving a death that is consistent with the values and expressed desires of the patient. Our hospice program offers you a full range of home care services to help you through this difficult time.

Our services include:

  • A full array of pain and symptom management
  • Social services
  • Counseling for patient, family and friends
  • Assistance with personal care, errands, and transportation
  • Medications, medical equipment and medical supplies
  • Respite care to relieve family caregivers
  • Inpatient care (if necessary)

Our caregiving professionals have been specially chosen for their sensitivity and experience in hospice care. When Home Hospice Care is the Right Choice
Hospice care is appropriate when your physician determines that the illness is terminal, that death will likely occur within six months and aggressive treatment has been replaced by comfort care. For most people, home hospice care at this time can provide a sense of peace and security, as well as practical assistance. If it becomes impossible for you or your loved one to remain at home, arrangements can be made for inpatient care.

Insurance Coverage for Hospice Care
Medicare, Medicaid, Blue Cross/Blue Shield, Blue Choice, Preferred Care and many private insurance companies offer coverage, although the level may vary depending on your insurance company and policy. Our Patient Service Representatives are available to assist you with insurance issues and help you plan for the financial aspects of hospice care.
